Each transport or refusal of services from Upper Saucon generates a Patient Care Report (PCR).  These health records contain confidential information and are not public documents.

If you would like to obtain a copy of an official PCR, please contact our Privacy Officer.  To receive a copy you must be the patient listed on the report* and be able to prove your identity with proper documentation.

There is a $20 fee for PCR copies to cover our processing costs.  This fee is payable with cash, money order or certified check.  The fee rate is subject to change without notice.

*PCRs may also be obtained by a parent, legal guardian or court subpoena.  See our Notice of Privacy Practices for complete details.
